QUALIFICATIONS

Education & Experience

A Master’s degree in Social Work, Psychology, or Counseling;
Clinician I: Registered as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), Associate Professional Clinical Counselor (APCC); and
Clinician II: Registered with CA-BBS as Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) credential; and
At least 1 year of experience providing services to youth and their families
Spanish speaking required

ESSENTIAL JOB FUNCTIONS

Position Specific

Functions as the lead case coordinator, providing comprehensive, strength-based, culturally competent, flexible, effective, quality mental health services, including psychotherapeutic interventions, to eligible at-risk youth, and families. Provides services based on clinical need and in consultation with supervisor to assigned youth, and families in the home, office, school, or other community based settings.
Coordinates and facilitates initial intake meeting with consumers meeting agency and County timelines and expectations
Responsible for the overall assessment, development, and implementation of Assessment Client Plans and Re-Assessment and Re-Authorization Client Plans
Provides and documents all Medi-Cal eligible billable and non-billable services, including but not limited to assessment, plan development, group session, collateral, rehabilitation, In Home Based Services (IHBS) case management, Intensive Care Coordination (ICC) and crisis intervention within required timelines
Ensures every family has an individualized safety plan that maximizes the use of natural supports and community based resources
Coordinates care with any external providers (i.e. psychiatry services, etc)
Attends all staff meetings, clinical supervision, and consultation meetings as scheduled
Provides services to youth involved in the Child Welfare Services, Juvenile Justice, and Mental Health systems, meeting all County requirements as listed in program scope
Participates in the agency on-call rotation
Participates in utilization, accreditation, peer review process as assigned
Responsible for meeting all outcomes as documented in Employee Development Plan
Models and communicates appropriate positive attitudes towards the agency’s Mission, Vision, and Values and program outcomes
Works collaboratively with all internal programs, departments, and staff to provide support as needed
Maintains positive relationships with consumers and customers
Coordinates with other services provided under the Medi-Cal model, acting in collaboration with other partners and ensuring documentation reflects standards that meet or surpass the agency, County Children's Mental Health, and Short-Doyle Medi-Cal standards

Starting Salary Range:

Clinician I: $75,504 - $81,400 DOE
Clinician II: $77,000 - $84,333 DOE
